Lost Certificate

• Certified copy of your ID
• An affidavit from a police station explaining what happened to the certificate
• Certified copy of proof of payment (R110):
  • Bank: ABSA

  • Reference: Rep followed by your student ID number

Diploma

• Completed and signed application form
• Certified copy of your ID
• Certified copies of N4-N6 certificates
• Logbook signed by your employer
